Preparing for Appointment

For the convenience of our patients, we have made new patient forms available to download in advance. These can be filled out ahead and faxed or mailed to the office, or brought in at the time of the visit.

Please contact our office if there are any difficulties with the forms.





For Our New Patients:
We have developed “Preparing for Your Appointment with Dr. Jafri”to ensure your visit with us is as pleasant as possible. It lets you know what you need to do before your appointment, what to bring with you, how to locate our office, and what to expect during your visit.

A Checklist for Your Visit

The following list will help you remember items and information to bring with you when you come for your visit.

  • Your insurance and/or Medicare cards
  • Current photo ID
  • Completed forms downloaded from our website, if convenient. These can be filled out on arrival at our office.
  • UCLA registration number if you have one.
  • Letter of authorization from your insurance carrier, if applicable (required by such HMOs as HealthCare Partners, Health Net and Kaiser)
  • Name, address, and telephone number of the doctor who referred you to the Dr. Jafri, if applicable
  • Prescription eyeglasses (even if they no longer correct your vision) or contacts as well as any written prescriptions for these.
  • Current list of medications, including supplements
  • Sunglasses (in case your eyes are dilated)
  • Although most people feel comfortable driving after dilation, we recommend that someone who can drive accompany you for your comfort.
  • If you are unable to keep your appointment, please call our office to cancel it so that the time may be given to another patient.

Length of Your Visit

The length of patient depends on the complexity of your eye problem. The initial visit is usually complete checkup, so be prepared to spend almost two hours at the office. (If your condition requires special testing or immediate treatment, your stay may be longer.)

Medical Care
As our patient, you receive the finest in eye care. Specialized testing and diagnostics may be ordered by Dr. Jafri.

Food Service
There are numerous restaurants and cafeterias near the office and in Westwood Village. If you have diabetes or hypoglycemia, you are welcome to bring snacks with you to your appointment.


Payment for Services
Professional fees vary with the nature of the clinical problem. Our staff can help you prepare. It is customaryto pay for services rendered at the time of your visit unless other arrangements have been made. Professional fees generally cover physician services only and do not include the cost of special diagnostic tests you may require.Charges for such tests are billed separately.

Dr. Jafri accepts many types of health insurance. However, it is difficult to predict what, if any, insurance benefits will be paid by individual insurers. Since you are responsible for charges (deductible, coinsurance payments, etc.) not covered by the insurance company, we recommend you check with your employer or insurance plan regarding benefits. If you are a member of an HMO, you must receive prior authorization from your insurer before making an appointment with Dr. Jafri in order to receive insurance coverage. Of course, you can choose to see Dr. Jafri privately.


Directions to the Office
Conveniently located within Westwood at the cornerof Westwood Boulevard and Wilshire Boulevard, our office is easily accessible from all points in Southern California. It is approximately 15 miles from the Los Angeles International airport. Street metered parking is available. Parking is available at the building for a fee.

San Diego Freeway (405)
Take Wilshire Boulevard offramp east (toward Westwood Boulevard). Turn left onto Gayley from Wilshire. The first building on your right is our building, as is the entrance to parking.

From Los Angeles International Airport (LAX)
Take the San Diego Freeway northbound to Wilshire Boulevard east, and continue as described above.
